Useful Terminology for this Episode

  • Weight Loss Maintenance: The sustained keeping off of weight that has been lost. Often defined in research as maintaining a ≥ 5% reduction in initial body weight for at least one year.
    • Success in weight loss maintenance is typically measured by the extent of weight kept off over the long term rather than short-term loss.
  • Metabolic Adaptation: Physiological changes following weight loss that reduce energy expenditure and increase hunger, favoring weight regain. This includes a drop in resting metabolic rate and shifts in appetite-regulating hormones (e.g., ↓leptin, ↑ghrelin), making weight maintenance harder than initial loss.
  • Behavioral Drift: A gradual relaxation of adherence to healthy behaviors over time. After weight loss, people may slowly slip back into old habits (larger portions, less activity, etc.), which can lead to weight regain if not recognized and corrected.
  • GLP-1 Receptor Agonists (GLP-1 RAs): A class of medications that suppress appetite and slow gastric emptying to aid weight loss.
    • Examples include semaglutide and liraglutide. These drugs have recently been approved for obesity treatment due to their significant weight reduction effects.
  • Food Noise: A colloquial term used to describe persistent background thoughts, cravings, and impulses related to food. Individuals with high food noise experience frequent urges to eat. GLP-1 RAs are noted in this episode to dramatically reduce “food noise”, helping patients experience normal satiety signals and fewer obsessive food thoughts.
  • National Weight Control Registry (NWCR): A long-running research registry tracking people who have successfully maintained significant weight loss.
